According to a new Human Rights Watch report, yeah, we f***ed up. “Interviews with locals and photographs and video of the building in question prove it was a well-known mosque that hosted lectures every Thursday between sunset and evening prayers,” CNN reports. Ole Solvang, deputy emergencies director at Human Rights Watch, added in a statement that “the U.S. seems to have gotten several things fundamentally wrong in this attack” and “U.S. authorities need to figure out what went wrong, start doing their homework before they launch attacks, and make sure it doesn’t happen again.” Welp.

Cleveland Facebook Killer Steve Stephens—who was charged with Sunday’s aggravated murder of 74-year-old Robert Godwin and sent the nation on a widespread manhunt—has been found dead. According to NBC News, Pennsylvania State Police confirmed the news around 11:30 a.m. on Tuesday. Police say Stephens committed suicide with a handgun after a short traffic pursuit that began in a McDonald’s parking lot. Facebook CEO Mark Zuckerberg said today at the annual F8 developer conference, “Our hearts go out to the family and friends of Robert Godwin Sr. We have a lot of work and we will keep doing all we can to prevent tragedies like this from happening.”
